I remember hearing there are some clipping issues with wider body shapes. It's Ok for NPC contacts, that just stand there, but with the action poses the heroes do wider bodies wouldn't work right.
As for the shoes, there are options, but you have to dig for them. A combination of jeans and sneakers for example is in there if you put it together right.
